Extravascular implantable cardioverter defibrillaton (EV-ICD) is an alternative option for patients where traditional transvenous ICD is not ideal or feasible. For single-chamber EV-ICD, a battery-powered pulse generator is implanted in a pouch under the skin near the serratus muscle, found on the ribs on the left side. The electrode runs from the pulse generator to the anterior mediastinum, the front part of the chest between the lungs. If an abnormal heart rhythm is detected, the device will deliver an electric current to the heart, known as defibrillation, to restore a normal heartbeat.

Ventricular arrhythmias are abnormal heartbeats that originate from the lower heart chambers, known as ventricles. These types of arrhythmias cause the heart to beat too fast, which prevents oxygen-rich blood from circulating to the brain and body and may result in sudden cardiac death. The most common types of ventricular arrythmias are ventricular tachycardia and ventricular fibrillation.
